Staff Challenge: National Patient Safety Goals

Hospitals are permitted to define what medication information is necessary to be obtained from the patient in a non-24-hour setting. What information do we collect in our outpatient clinic and ED?

•    How often it is taken
•    What dosage
•    Route
•    Why the patient is taking the medication
