From the AHAP Blog: The first entry from the AHAP Conference Contest

The AHAP blog is running a contest that awards one winner each month a free registration to the AHAP Conference in May.

The first entry for February comes from The Quality Department of Regional Medical Center of San Jose (CA). They sent in the following sample documents as their entry into the AHAP registration giveaway.

They wrote:

The Quality Department in our facility is responsible for tracers, core measures and peer reviews among other things. Below are two tools which are really very helpful in performing these jobs on a regular basis. The tracer documentation tool is a tool we use to trace documentation compliance only, a separate one for clinical aspect is used when interviewing the nurses. Our clinical specialists usually do this part. Another really very useful tool for us is our core measure 101 brochure which we give out in every new employee orientation, and to the nursing and medical staff who need core measure education.
